Position: Speech Therapist Location: Pocatello, ID Job Id: 3026-ID PT of Openings: 1 Speech/Language Pathologist - Part Time Monte Vista Hills Healthcare Center - Pocatello, Idaho About the Opportunity: Monte Vista Hills Healthcare Center of Pocatello is offering a wonderful opportunity for a Speech Language Pathologist who desires a flexible part time schedule. Your schedule can be 4 or 5 days per week and up to 4 hours each day with opportunity to grow into full time in the future. If you consistently work 19 or more hours per week we can also offer a paid time off and 401K plan package. We are currently developing an outpatient Parkinson-'s program and will pay for you to be LSVT LOUD certified. In addition, you will love our fun in-house team of therapists who find amazing ways to celebrate operational and clinical success We can consider a recent graduate and provide CF supervision, although there will not be a full time supervising SLP on site. Monte Vista Hills receives support from Ensign Services who provides a wide range of clinical and operational assistance to over 200 independently owned healthcare operations located throughout the United States. Duties: Evaluate patients within 48 hours of physician referral. Develop effective treatment plans and obtain approval for services from referring physician. Treat patients according to the physician-'s treatment plan and adhere to facility documentation guidelines. Communicate with supervisor and other health team members regarding patient progress, problems and treatment plans. Participate in Patient Care Conference, Utilization Review Meeting and Rehabilitation Conferences. Participate in in service training program for other staff in the facility. Record evaluations, daily treatment notes, weekly progress notes, HCFA 700 & 701 forms per documentation guidelines. Record treatment charges per facility policy and procedure. Instruct patient's families or nursing staff in maintenance program. Participate in discharge planning. Secure necessary durable medical equipment and adaptive equipment for patients to facilitate independence. Act as a clinical preceptor for affiliating CFY / RFP Speech Language Pathologist and student programs. Comply with the American Speech Language and Hearing Practice Act for the state, and the facility policy and procedures.
