We are looking for a dynamic and passionate Advanced Practice
Practitioner to join our team! Responsibilities What You'll Do The
Advanced Practice Practitioner provides patient care services by
evaluating and treating patients within the scope of individual
authority granted by the Idaho State board of Medical Examiners
and/or collaborating physician. Clinical responsibilities include
comprehensive evaluations, medical management, therapeutic
interventions and patient education and discharge summaries.
Reports to and receives functional guidance and direction from the
delegated supervisory physician for clinical matters. May receive
direction for administrative matters from the Office Manager. The
successful candidate will be able to: Assessment - Collects data;
Physically asses patients; Patient history completed, evaluated and
signed within required time frame; Completes medicine
reconciliation, admission, transfer and discharge orders as
directed by physician Implementation - Directs and coordinates care
of patients; Identifies patient and family learning needs;
Follows-up on outstanding labs, tests etc as directed by physician;
Follows-up calls to patient, family and or referring providers as
directed by physician Plan - With appropriate input from all team
members, including patient, family, or significant other;
initiates, implements, and documents a patient care plan based on
standards of nursing practice, policies, and procedures,
established protocols, and professional judgment; Utilizes
admission history and routine reassessments in planning care; Plan
of care updated per department standard or as status changes/goals
met/issues resolved; Initiates collaborative discharge planning;
Discusses plan of care with patient and/or significant other
Evaluation - Evaluates effectiveness of interventions and redirects
plan of care as condition indicates; Assures that patient care
interventions and activities are directed at the resolution of
identified patient problems/needs; Performing perioperative risk
evaluations in the pre-surgery clinic Documentation - Proficient in
computer programs used in providing/documenting care; Documents
assessment, plan of care, interventions, and patient response
consistent with hospital and unit standards; Documentation is
legible and complete; All patient forms are complete; Uses
appropriate/approved abbreviations Delegation and Coordination -
Evaluates and delegates tasks according to patient needs and skill
level of caregiver; emonstrates organizational skills (completes
work within shift, evaluates work progress throughout shift,
prioritizes); Provides timely and adequate supervision to hospital
personnel as required based on staff qualifications and competence;
Based on needs assessment, coordinates with all disciplines
involved in the care of the patient Process
Improvement/Communication - Reports and addresses real or potential
problems with quality and appropriateness of care through the use
of occurrence reports, or other appropriate means; Contacts
physicians in a timely manner; Follows appropriate chain of
command; Notifies manager of issues in a timely manner ; Uses
judgment, tact, and diplomacy to effectively resolve conflicts;
Maintains consistent and effective communication with physicians,
other members of the healthcare team, patients and families Heart
Failure program: Pre & post and Intra hosptial management.
Qualifications What You'll Bring: Current licensure as a Clinical
Nurse Specialist or Physician Assistant in the State of Idaho.
Current Idaho Board of Pharmacy Controlled Substance licensure
Current BLS Two plus years of Rheomatology experience preferred
